Forms

How to build a form in the builder, publish it with a public link, embed it on your site, receive responses, export CSV and trigger email, webhook, task or event on each response.

The Forms app is for capturing leads, support requests, sign-ups and feedback without writing code. You design the form, publish it and share the link; each response is stored under Submissions and can trigger automatic actions.

Create and build

  1. Under Forms, click New form. A blank form is created as a Draft and opens straight in the builder. To start from a template, open Templates (categories General, Marketing, Events, Support, Feedback and HR) and use Use template.
  2. Give it a name in “Form name”. The left panel groups fields into Common, Choice, Number & date and Advanced: Short text, Email, Phone, Long text, Number, Select, Multiple choice, Checkbox, Date, URL, File and Hidden.
  3. Add fields with Add field. Click a field on the canvas to edit it in the Edit field panel: Label, Name (key), Help text, Required, default value, min and max, Max length and Options (value and label). Duplicate and Remove field are there too.
  4. The Saving… / Saved / Save failed indicator shows autosave; Save draft forces a save.

Under Settings you adjust the Submit button label, the Thank-you message, an optional Redirect URL and the Honeypot anti-spam, a hidden field that gives bots away. Multi-step splits the form into steps with Add step, and the Layout can be Minimal, Card, Dark, Wizard or Inline. The Preview tab is a preview in the browser; to see the real renderer, publish.

Publish and share

Publish changes the status to Published; Unpublish goes back to draft and Archive removes it from the main list. A published form lives at forms.<domain>/p/<slug>, a standalone page anyone can open without logging in. The address (slug) is generated automatically.

Embed shows three ways to use the form (only after publishing):

  • JavaScript (recommended): paste the <script src="…" async></script> tag where the form should appear; it resizes itself.
  • Raw HTML: no CSS, so you style it on your site.
  • Direct link: for social media and email.

Receive responses

Under Submissions (per form, or the view of all the team’s responses) you see the received date, the content and, under Incomplete, what someone started filling in and did not submit. Download CSV exports the responses and Delete erases a response, with no way back. Deleting a form is reversible for 30 days.

Public submission has protections: at most 10 submissions per minute per IP address and form, the honeypot and, if the installation’s operator configures them, reCAPTCHA v3 and Cloudflare Turnstile. When a response is flagged as spam, the sender still sees success, but the response is marked and triggers no action.

Actions on each response (triggers)

The builder’s Triggers run something when a response arrives. The types are Email (notifies one or more people), Webhook (sends to Zapier, Make or your own backend, with an optional signature in the X-Axuz-Signature header), Calendar Event (books an appointment) and Workspace Task (opens a task on the board). The last two ask for identifiers (UUIDs) of the workspace, the board and the column, which you copy from the addresses of the corresponding screens. If an action fails, or if the destination service is not configured in the installation, the response is already saved and stays valid; only the action does not happen.
